What are LAMPOON and BASILICA?
LAMPOON (Laceration of the Anterior Mitral leaflet to Prevent Left Ventricular Outflow Obstruction) and BASILICA (Bioprosthetic or native Aortic Scallop Intentional Laceration to prevent Iatrogenic Coronary Artery obstruction) techniques are procedures in which valve structures are cut using electrosurgical methods before transcatheter valve replacement (TAVI, TMVR). These techniques are used to prevent obstructions that may occur during valve replacement.
LAMPOON Technique
The LAMPOON technique is a procedure in which the anterior leaflet of the mitral valve is cut using an electrosurgical method before transcatheter mitral valve replacement (TMVR). This procedure is performed to prevent left ventricular outflow tract (LVOT) obstruction during placement of the new valve.
BASILICA Technique
The BASILICA technique is a procedure in which the leaflets of the aortic valve are cut using an electrosurgical method before transcatheter aortic valve replacement (TAVI). This procedure is performed to prevent coronary artery obstruction during placement of the new valve.
How Do They Work?
Both techniques are performed using special electrosurgical devices delivered to the heart via catheter. The procedure is performed under general anesthesia, accompanied by echocardiography and fluoroscopy. The valve leaflets are cut in a controlled manner to create a suitable area for placement of the new valve.
Who Is It Suitable For?
- Patients planned for transcatheter valve replacement
- Patients at risk of obstruction during valve replacement
- Patients with suitable mitral or aortic valve anatomy
- Patients with high surgical risk
Advantages
- Precise cutting with electrosurgical technology
- Reduces valve replacement complications
- Prevents left ventricular outflow tract obstruction
- Prevents coronary artery obstruction
- Increases procedure success
After the Procedure
LAMPOON and BASILICA procedures are usually performed in the same session as the valve replacement procedure. Regular follow-up after the procedure is important. These techniques are important techniques that Prof. Dr. Teoman Kılıç first applied in Turkey and contributed to world literature.
