PARAVALVULAR LEAK CLOSURE

MITRAL PARAVALVULAR LEAK CLOSURE WITHOUT OPEN HEART SURGERY

Mitral prosthesis valve is localized between left atrium and left ventricle of the heart. There are two ways to cross the hole across the mitral prosthesis. First way is named as the antegrade method and the second way is named as retrograde method.

a)Antegrade method
The veins across the groin take dirty blood from legs to right atrium. We make little punctures by a small needle to the femoral vein and put a sheath in it at our catheterization laboratory.
Then, we send our little catheters from this sheath in to the right atrium. We made a transseptal puncture to the atrial septum which is a layer between right and left atriums. Then, we passed from right to left atrium.

After we put our catheter or guidewire to the left atrium, we send a special steerable catheter to the left atrium. By using this catheter, we can localize the defect and we send some little catheters and a special guidewire within the steerable catheter to cross the paravalvular leak. After passing through the leak, we put the catheter into the left ventricle and we send a snare from the other side of groin artery. Then we catched the guidewire in the left ventricle and create an arteriovenous wire loop.In our clinic, we can also close the defects by antegrade method without arteriovenous wire loop.

After creating an arteriovenous wire loop, the delivery system of the closure device is sent from the femoral vein. When we send the delivery system to the left ventricle, we open the distal part of the device and pull back the system and the device to the defect. We open the waist segment of the device in the defect and open the other part in the left atrium and close the defect. After the procedure, endothelization develop across the device at 6 month or 1 year.

b) Retrograde method
The groin arteries are the main vessels for us to reach to the aorta and to the left ventricle. We make little punctures by a small needle to the femoral artery and put a sheath into the femoral artery at our catheterization laboratory. We send our little catheters from this sheath in to the left ventricle. Then we cross the paravalvular leak by a special guidewire. After passing through the leak, many centers create an arteriovenous wire loop. However, we are closing these defects without creating an arteriovenous wire loop. We believe that our method can reduce the procedure time, radiation dosage, costs and complications. We recently published our important method in the Texas Heart Journal.After passing through the leak, the delivery system of the closure device is sent from the femoral artery.

When we send the delivery system to the left atrium, we open the distal part of the device and pull back the system and the device to the defect. We open the waist segment of the device in the defect and open the other part in the left ventricle and close the defect. AORTIC PARAVALVULAR LEAK CLOSURE WITHOUT OPEN HEART SURGERY

Aortic prosthesis valve is localized between aorta and left ventricle of the heart. To crosss the paravalvular leak across the aortic prosthesis, we usually use the femoral arteries of the groin.
The groin arteries are the main vessels for us to reach to the aorta and left ventricle. We make little punctures by a small needle to the femoral artery and put a sheath into the femoral artery at our catheterization laboratory. We send our little catheters from this sheath in to the aorta. Then, we cross the paravalvular leak by a special guidewire.

After passing through the leak, the delivery system of the closure device is sent from the femoral artery. When we send the delivery system to the left ventricle, we open the distal part of the device and pull back the system and the device to the defect. We open the waist segment of the device in the defect and open the other part in the aorta and close the defect. After the procedure, endothelization develop across the device at 6 month or 1 year.



FAQ
Kapalı yöntemle yapılan tedaviler sırasında ortaya çıkan istenmeyen durumlar, açık cerrahiyeoranla çok daha düşüktür. Bu tedaviler sırasında, açık cerrahi sırasında ortaya çıkabilecek ciddi kanama, uzamış anestezi komplikasyonları, damar ve boşluk yaralanmaları gibi komplikasyonlar sıklıkla görülmemektedir.

Göğüs kafesi yarılmadan kasıkta sadece küçük bir iğne ve kılıf yerleştirilmesi ile gerçekleştirilen bu operasyonların kozmetik üstünlüğü nettir. Ancak, her operasyonda olduğu gibi bu işlemler sırasında da kanama, hava embolisi, kalp zarı etrafında kan toplanması gibi istenmeyen olaylar çok nadir de olsa ortaya çıkabilir fakat açık cerrahiye kıyasla bu durumların ortaya çıkması son derece nadirdir.

Özellikle daha önce açık cerrahi uygulanmış bazı hasta gruplarında, ikinci kez yapılacak bir açık cerrahi sırasında, kalbe ulaşım sırasında birçok fibrotik doku ve yapışıklıklarla karşılaşılmaktadır. Aynı hastada tekrar göğüs kafesinin açılması, perikard yapışıklıkları, çevre dokuların yarattığı bağ dokusu iyileşmeleri gibi nedenler; ikinci kez cerrahi yapılmasını zorlaştırmakta, anestezi ve kardiyopleji süreleri uzamakta ve operasyon sonrası hastanede ve yoğun bakımda kalış süresi uzayarak iyileşme süresi gecikebilmektedir.
Perkütan transkateter tedavi yöntemlerinde kullanılan cihazlar, birçok hasta grubunda test edildikten sonra kullanıma girmiştir.

Özellikle kalp deliklerinin kapatılmasında kullanılan cihazların çoğu nitinol olarak adlandırılan hafızası olan özel metallerden üretilmiştir. Nikel alerjisi olanlar dışında cihazların herhangi bir yan etkisi bildirilmemiştir. Bu tip durumlarda da platin kaplı cihazlar kullanılabilmektedir. Bazı uygulamalar sırasında, eğer uygun ölçüm yapılmamışsa, uygun cihaz seçilmemişse cihazlar yerinden fırlayıp kalp boşlukları içerisine kaçabilir. Ancak bu tip durumlar için cihazları vücut içerisinde yakalayabilecek özel kementler üretilmiştir ve cihazlar bu kementler ile yakalanıp vücuttan çıkarılabilir.

Mitral balon valvuloplasti, aort ve pulmoner balon valvuloplasti gibi kapak darlıklarının balonla açılması işlemlerinde kullanılan balonlar, nadiren de olsa çevre dokulara ve kasık damarlarına zarar verebilir. Ancak açık cerrahiye kıyasla bu tip istenmeyen olaylar oldukça nadirdir.

Transkateter aort kapak implantasyonu için kullanılan kapaklar, kalbin ileti sistemine bası yaparak kalıcı kalp pili gereksinimine neden olabilir. Bazı hastalarda kapak takıldıktan sonra kapaklar etrafında paravalvuler kaçak ortaya çıkabilir. Bu açıdan hastaların belirli aralıklarla takip edilmeleri oldukça önemlidir.

Tüm kapalı tedavilerde çok nadir de olsa vasküler ve nörolojik olaylar ortaya çıkabilir. Ancak açık cerrahiye kıyasla bu tip olayların sıklığı çok daha azdır.
Kapalı tedaviden sonra tüm hastalar 1 gün süre ile yoğun bakım ünitesinde izlendikten sonra, istenmeyen bir durum mevcut değilse ertesi gün servise alınır. Servis izlemleri sırasında herhangi olumsuz koşul gelişmemişse işlem sonrasında kullanacakları ilaçların tedavi dozları da ayarlanarak hastalar, iki veya üç gün içerisinde taburcu edilir.
Kapalı tedaviden sonra tüm hastalar, 1, 3, 6 ve 12. Aylarda düzenli olarak kontrollerini yaptırmak zorundadırlar. Birinci yıl sonrasında da her 6 ayda bir düzenli kontrol yaptırmalarında fayda mevcuttur.
Kapalı yöntemler sırasında mutlaka kullanmak zorunda olacağınız ilaçlar olabilir. Bu ilaçların hangi sıklıkta ve ne kadar süre ile kullanılacağı, ne zaman kesileceği konusunda mutlaka doktorunuza danışmalı ve düzenli olarak takiplerinizi yaptırmanız gerekmektedir.
